This form can be used to help get rid of unwanted doorstep vaccine pushers. Because if they are unwilling to sign the form then it means they are not willing to put their money where their mouth is. 

And if they are willing to sign they'll have to go find 2 witnesses and provide proof of address which they'll probably not be willing or able to do. 😇

This could be a useful tool to print in advance and give the elderly who do not want the vaccine and who happen to open the door to a vaccine pusher on their doorstep.
